Community Support
All of CareLink’s services embody the principles of psychosocial rehabilitation, which promote achievable outcomes and community integration. Training in social skills and career skills is offered along with life skills training to help our program participants meet this goal. Emphasis is also placed on participation in activities and community events, which enhance resident lives and promote socialization and familiarity, as well as integration with the local community.
Connecting With Local Resources
CareLink’s Mobile Psychiatric Rehabilitation (MPR) services assist adults with mental illness in developing the skills and supports necessary to lead more self-sufficient, personally satisfying lives.
MPR services are designed to be short-term, goal oriented, highly individualized, and directed by the participant; individuals receive assistance in learning the particular skills they need to transition to or maintain community living.
In the MPR program, participants identify a specific area of need. An MPR Specialist conducts an assessment with the client to identify strengths, as well as personal and community resources. Together, they develop a service plan that outlines the tasks needed to reach the identified goals, and both parties have responsibilities between visits to make progress on the goals.
Targeted Outcome Examples:
Acquire skills in nutrition, budgeting, self-care, or use of public transportation
Use time productively at home and in the community
Manage mental health issues such as symptoms and medication
Use resources such as employment specialists, program participant groups, advocacy groups, self-help or other community support
Enjoy the support of family, friends, and coworkers
Better coordinate their care among service provider and natural supports
