Transition Age Services address the educational, vocational, and community living needs of 18-22 year olds. Young adults who experience mental illness often find themselves caught between systems of care that aren’t designed to meet their needs. CareLink’s Transition Age services recognize the complex challenges that mental illness and co-occurring disorders present to finding one’s place in the community. Our flexible array of psychosocial rehabilitation services including residential services, care management, and family outreach can make it possible for people to reach their goals for stable, satisfying community living. Who will benefit? The program serves young men and women ages 18-22 from Delaware County who have a mental illness. Often people needing services will also experience co-occurring disorders such as substance abuse, mental retardation, autistic spectrum disorders, adjustment problems, medical disorders, hearing and vision or other physical impairments. Program Services and Supports The Transition Age Program ensures that the individual interests, needs, goals, and cultural and ethnic preferences of each person are reflected in the services they receive. Services emphasize the development of living skills, social skills, vocational/education skills, and constructive leisure/recreation activities. Learning takes place in the context of the community so that treatment activities have direct application. CareLink’s Transitional Age residential services employ a team of staff composed specifically to address the needs of the young people to be served. We are a vital part of each person’s network of care. Our staff coordinates services closely with family, psychiatrists, juvenile justice, probation and parole, medical professionals and other resource providers. Services are not time limited. The program is licensed as a CRR adult program in Delaware County under Pennsylvania’s Department of Public Welfare regulations. Residential services
Attention to work and school We provide individualized employment, education, and training services to promote self-worth, the development of natural supports, and community integration. Job placement in itself is not our goal. Teaching people to help themselves by expanding their existing skills is the key to success. We use individualized career counseling and offer a variety of skills training options to teach and build needed employment and education supports. Attention to recreation and leisure Family involvement Family and/or significant others are invited to participate in treatment team planning and progress reviews.
