CareLink’s Mobile Psychiatric Rehabilitation (MPR) services provide one-to-one direct skills training and development of community resources to adults with serious mental illness. MPR services are highly individualized and directed by the participant. People get help to learn the particular skills they want to transition to or maintain community living. Just the services you want In the MPR program, people identify a specific area of need. An MPR Specialist conducts an assessment with the client to identify strengths, and personal and community resources. Together they develop a service plan that outlines the tasks needed to reach the identified goals. Both parties have responsibilities between visits to make progress on the goals. The services are tailored to what the program participant wants and needs. The MPR Specialist meets with the participant in his/her home, work, school or neighborhood. This helps to make sure the services fit each individual’s cultural background, natural environment, and special needs. Services are designed to be short term and goal directed. The MPR program’s focus is on personal choice, and developing the skills and supports needed for more self-sufficient, personally satisfying community living. Service Area and Eligibility CareLink’s MPR services are available to residents of Delaware County, PA, who have a serious mental illness. People are referred for MPR services through the Psychiatric Rehabilitation Assessment Specialist at the Northwestern Human Services or Crozer Keystone Base Service Units.
