| Residential Nonresidential | Community Options offers outreach support and case management for people with disabilities CareLink’s Community Options program provides one-to-one outreach and case management for people with disabilities including mental illness, physical disabilities, and substance abuse problems. This program increases people’s independence, self-sufficiency, and quality of life in the community. Maximizing independent living Community Options services are driven by the unique needs, interests, and goals of each person served. The program provides skill teaching and supportive services to help people live as independently as possible in the community. Case managers help each person have the specific support and training they want and need. Goals are identified and steps are outlined to teach necessary daily living skills and secure other needed services to maximize community living. Services include:
Eligibility and service area The Community Options program serves adults with any type of disability including mental illness, HIV, physical disabilities, developmental disabilities, substance abuse problems and homelessness. People in Wilmington, Newark, and throughout New Castle County, Delaware are eligible. People are referred through an extensive network of community agencies, social service providers, churches and through family and self referrals. Community Options is funded through the United Way of Delaware, New Castle County Community Development Block Grants, the City of Wilmington, HUD, DE Division of Aging, DE Grant-in-Aid, private grants and the generosity of our contributors.
