| Residential Nonresidential | Psychiatric Rehabilitation Assessment Specialists conduct an evaluation of a person’s interest and needs Delaware County has two Psychiatric Rehabilitation Assessment Specialists. They are the point of entry for people with serious mental illness into any of the psychiatric rehabilitation services in Delaware County. The assessor makes referrals and ensures the connection with day rehabilitation, vocational/educational, Mobile Psychiatric Rehabilitation and clubhouse programs throughout the County. CareLink’s Assessment Specialist is located within the Base Service Unit of Northwestern Human Services/ Life Guidance Services in Sharon Hill, PA. What is the Psychiatric Rehabalitation Assessment? The assessment is a meeting with the Assessment Specialist to identify the person’s skills, needs, interests and readiness for rehabilitation. It is designed to identify the special interests and needs of the individual, including how the person’s cultural and ethnic background may influence his or her preferences and goals. A recommendation is made based on the best match for services and then a referral is made to that program. The Specialist is responsible for providing any outreach and tracking that is necessary to ensure accurate placement with the identified services. Who is it for? Individuals who are at least 18 years of age, residents of Delaware County, and have a serious mental illness are eligible.
